Helping Patients Face a Serious Illness: A Workshop for Primary Care Providers
In this 3 hour workshop, Drs. Hsien Seow and Samantha Winemaker will offer a more in-depth look at the 7 keys for navigating a life-changing diagnosis and share how these keys are relevant for health care providers. They will provide simple and practical actions, tools, and resources to better support health care providers to offer an early palliative care approach to their patients and bridge communication between the patient/family and health care provider. Providers will be reminded of the importance of person/family centred care and the surprising innate benefits beyond those for patients and families.
Specifically, by the end of the workshop, providers will appreciate the following:

How to guide their patients and families through an illness journey by improving illness understanding

The role of the family as part of the care team and how to work together as one team.

How to illicit patient and family readiness for realistic illness conversations and revisit the importance of patient/family centred care.

How to meet the recommendations as outlined in the province’s Community Model of Care by balancing hope and planning, early in an illness with all their patients.
